I’ve never had to get an AHC as Nix has an EU Pet Passport, but apparently it’s page 4 of the AHC that needs to be completed for the tapeworm treatment.

Make sure they enter the date and time, sign and stamp it. Also check they’ve entered the microchip number correctly. Ours didn’t but I checked it before leaving the car park and asked them to add the missing digit.
