We used http://www.southdownsmotorcaravans.co.uk/ you need to give them the fridge make and serial no.Very good service as part of our serial no wiped off from the plate but still managed to send correct shelf.

If it's a Dometic 8000 series they always come with one missing unless you have a real posh van.
About £45, I made one from 6mm acrylic sheet

We had one break in our last fridge at home the boss put a le creuset caserole dish on the glass shelf while still hot......bang!!!!. Got one cut at the local glass suppliers and the edge chafered cost a lot less than one made for the fridge still ok when it gave up the ghost.
