Not an expert but sounds like throttle position sensor, I would have the fault codes read for peace of mind 👍😁

Not all faults put a light on but would be recorded, sometimes it’s something simple like touching the brake and accelerator at the same time that confuses things, it definitely wants the fault codes reading, for now wipe the exhaust pipe end to make sure it’s clean, I’m assuming it’s twin egr engine or is it adblue ?

Now funny you should mention "touching the brake and accelerator at the same time" I Googled the problem this morning & come across a post in a 'Fiat ' forum where a chap had the same thing happen on his Ducato 2 separate Fiat Dealerships told him by doing this puts the Van in 'safety mode' he says he didn't think he had touched both as do I, BUT, I have had our Van on Diagnostics today & apart from 1 other issue logged in (pass w/dow relay) there was a logged 'Brake & Accelerator Battery' issue. So maybe Fiat are correct , Faults all cleared & Van seems OK, 👍👍👍
