I've read quite a bit about applying to hold on to my C1 in June. I'm wondering whether anyone knows of a straight forward tick list which will get me through the paperwork without falling off my horse. I have a tendency to wander all over the way without having tick lists.
 
Get the sight test signed when you have your free Eye test, much easier and quicker.

I dont think there is a definitive tick list other tban the guidance booklet that comes with the D4 form when supplied by DVLA....


The guide further down this webpage, tells you how the form needs to be completed.

Firstl get forms D2 and D4. D2 has to be obtained through a main PO or by post from DVLA because it has a plasticised section and cannot be downloaded from internet. D4(medical) can be downloaded. This far in advance it may be easier to request whole C1 pack from DVLA. That can be done on-line.

Now timing: Doctor can fill-in D4 up to 4 months before licence expiry. Application with both forms cannot be earlier than 3 months before expiry.

For Medical one can go to any UK, or EU, qualified doctor. Most can do the eye test too, but check with them in case a separate Optician's eye test and signature is needed. Reports on here indicate that local GPs charge a lot compared with specialist companies like D4Drivers, who will do the test at local venues, such as hotels and for £50-60. Phone them for venues and an appointment.

Get the Medical done within the 4 months, as above. Check that the doctor has answered the questions correctly - some questions are asked in the negative, so need careful answers.

So now you have the D4 and only the D2 to complete. Just go through it methodically You are applying for a lorry licence (and bus if you have D1 +D1E). Most of the form instructs you to skip big chunks.

The rules about whether you have to attach a photo in the sticky section are a bit confusing - easiest is to attach one anyway.

Over 70 there is no fee to pay.

I have renewed 4 times now and no problems, even using a Polish doctor in Poland to fill-in the D4.
