D4 form...do you send any supporting documents?

I'm about to fill in my D2/D4 form to renew license and hopefully keep the C1. Is it advisable to send in any supporting documentation such as past letters from consultants? Granted that these could (will?) be lost by the DVLA but I'm wondering if they would help the application. Seems like they will contact my doctor anyway (have heart problems) and I expect to have to do the treadmill test.
 
Following this thread....
I had a notion that you will need a medical test (your GP or there are various commercial companies which can organise this with their GPs) and that was it....?

Yes I asked at my surgery today and they said they would respond to any request from the DVLA but didn’t offer any assistance with the D4 form. I’ll go to D4drivers for that. I’m expecting a long drawn out process and not planning any time abroad until next spring!

Hi. I recently applied and included a hospital letter at suggestion of D4D doctor. However I now understand that this will not influence application so suggest send form only and DVLA will arrange Consultant referral if indicated from the information on D4 form.

On my last application, the Dr. told me to include the letter I had received from my Consultant in 2000.
She said it was mitigation that I should continue obtaining the C1.

Apparently, whatever is wrong with my dickie ticker will not cause me to keel over suddenly.

I understood the majority of the D2/D4 forms had to be completed and signed off by a Doctor at least. I have used D4 for drivers previously and they completed both the eyesight and medical side on the forms and I just added my details and signature. (y)

Re your question of adding additional info .if you do just supply copies and the dvla can then verify those with whoever doctor issued them.probably more quickly than just going back to your gp. Just a thought
